Huther Doyle is the Lead Health Home in the Finger Lakes Region for HHUNY (Health Homes of Upstate New York).
What is a Health Home, you ask? Health Homes provide Care Management services to help clients improve their health. How do we do this? Our Care Managers work with clients to identify their health goals, coordinate their medical care, advocate on their behalf, and connect them to resources to find affordable housing, food, transportation, etc.
To qualify, you must be a Medicaid recipient residing in the Finger Lakes area and meet at least one of the following conditions:
Two or more chronic health conditions, such as asthma, diabetes, heart disease or a Substance Use Disorder.
Have a significant mental illness.
Live with HIV/AIDS.
