An Extensive Network of Support
There’s No Place Like a Health Home
That’s because a Health Home isn’t a place—it’s an extensive network of support for Medicaid recipients in the community. Our Health Home care managers coordinate all the services a person needs to stay healthy and out of the hospital or emergency room.
Who is eligible?
Medicaid recipients, including those who are already members of a managed care organization and who meet at least one of the following conditions:

Struggle with two or more chronic health conditions, such as asthma, diabetes, heart disease, a mental health condition or substance use disorder

Have a significant mental illness

Live with HIV/AIDS

Reside in any of the following New York counties: Allegany, Cattaraugus, Cayuga, Chautauqua, Chemung, Cortland, Erie, Genesee, Livingston, Madison, Monroe, Onondaga, Ontario, Orleans, Oswego, Schuyler, Seneca, Steuben, Tioga, Tompkins, Wayne and Yates

Working with providers to coordinate your care

Advocating on your behalf so you receive the care you deserve

Navigating the complex healthcare system

Linking you to resources for finding affordable housing, food, clothing, childcare and transportation

Offering you more meaningful activities to improve your health

Helping you identify your own goals and ensuring everyone involved in your care understands the care plan you created

Ensuring you have access to the things you need to reach your goals
